Depth is the argument for the capital. The public stack includes Hospital Universitario La Paz (four linked hospitals at one northern campus), Hospital General Universitario Gregorio Marañón (1,671 beds; a reference centre for cardiology and oncology) and Hospital Universitario 12 de Octubre (1,256 beds) — bed counts as published, checked 8 October 2026. The centre runs on the Metro and buses; Barajas (MAD) is the country’s U.S. gateway. Iberia’s published August 2026 schedules show 99 weekly North America departures — New York JFK 14 a week and Newark 7 (daily since 29 March 2026), Miami 14, Boston 13, Washington–Dulles, Los Angeles and Chicago 7 each, Dallas–Fort Worth 6, San Francisco 4, Orlando 3 — and American flies Madrid–Philadelphia (schedules checked 8 October 2026; frequencies change by season). The U.S. Embassy in Madrid hosts the Federal Benefits Unit for Spain — the office that handles Social Security cases in-country.
English runs further here than anywhere inland: reliable at physician level in the private hospitals above, and familiar in the embassy district and the international schools’ orbit. The town hall, the health centre and the landlord still run in Spanish — Madrid is a working Spanish capital first, and daily life rewards the language.
The capital works at Spanish hours: coffee and toast at eight, the menú del día at two, tapas at nine.
