Málaga is on the short list of cities with large public university hospitals plus private hospital groups, and physician-level English is reliable in the coastal private system — the strongest English-language healthcare net of the cities compared here. The frame: Medicare pays $0 (Medicare.gov, April 2026); private no-copay cover in year 1 (about €170–€250/mo per person at 65 in broker tables); Convenio Especial after year 1 at €157/mo per person at 65+.
